Both glaucoma and multifocal intraocular lenses (IOLs) can create contrast abnormalities that can affect the accuracy of glaucoma visual field testing. Specific discussion and caution regarding the use of multifocal IOLs, therefore, is required in the setting of glaucoma. As a result, multifocal IOLs are not absolutely contraindicated, but they are not recommended in glaucoma patients because they will confuse the results of future, required serial, glaucoma visual field testing.
The use of steroids undoubtedly causes an increase in IOP, it may be necessary. The point is whether the severity of the glaucoma is identified, and then followed and managed properly. IOP is only one variable to be followed in glaucoma management. Other required serial testing (visual field testing, and OCT) also need to be performed.
