Handoff is the transfer and acceptance of patient care achieved through effective communication. It is done in real time with one provider passing patient specific information to another to ensure continuity and safety of care. Failed handoff and poor communication is a long-standing concern that leads to patient harm. Gaps in communication and inadequate handoff communication are contributing factors to adverse events including many types of sentinel events.
